• Comprehensive physician exams

• (H & P, op note, consult, discharge summary)

• Imaging studies

• Initially send the reports; the FAA may ask for images later.

• Pathology reports (cancer diagnoses)

• Laboratory studies

• (only those that have a positive or negative bearing on the chance of incapacitation)

• A current (w/in 90 days) medical report from the treating physician (if there are multiple docs, pick the one who's actually doing stuff now.
