- print out the FAA documentation requirements Locate your regional FAA Flight Surgeon's phone number
- Schedule a consult with your treating doctor.
Don't piggy-back 3 other issues into the visit!
- mail them to your doctor prior to your consultation bring along a second copy with you at the visit.

- Be clear to your doc that you need help with documents, not medical treatment at this point. Expect your doc to be a bit flummoxed, but can be really, really helpful in identifying which records will be meaningful to an aviation medical consultant.
